Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW PURE INFUSION MAY USE AND SHARE YOUR HEALTH INFORMATION, INCLUDING BY TEXT MESSAGE AND THROUGH A HEALTH INFORMATION EXCHANGE, AND HOW YOU CAN ACCESS IT. PLEASE REVIEW CAREFULLY.
OUR COMMITMENT TO YOUR PRIVACY
We are required by law to protect the privacy of your health information, give you this notice of our legal duties and privacy practices, abide by its terms, and notify you if we can’t agree to a requested restriction.
HOW WE USE AND SHARE YOUR INFORMATION
Treatment: with clinicians involved in your infusion care and your referring/prescribing providers.
Payment: to bill and collect payment via AdvancedMD and our clearinghouse, Waystar.
Operations: for quality assessment, staff training, licensing, and administration.
Other permitted uses: as required by law; public health and oversight activities; court/administrative orders; health information exchanges (see right); organ/tissue donation; limited research; safety threats; workers’ compensation; law enforcement in limited circumstances.
Your authorization required: for uses not described here, most psychotherapy notes, marketing, and any sale of your information. You may revoke authorization anytime in writing.
TEXT MESSAGE (SMS) COMMUNICATIONS
With your consent, we may text you appointment reminders, scheduling confirmations, and treatment-related alerts, and allow two-way replies with clinic staff.
Consent: given at intake or via the patient portal; not required to receive treatment.
Risk: standard SMS is not end-to-end encrypted.
Opt out: reply “STOP,” tell clinic staff, or contact us (below). Message/data rates may apply.
HEALTH INFORMATION EXCHANGE
Pure Infusion participates in Contexture, a regional health information exchange that lets your care team securely access records such as medications, labs, allergies, and visit history, improving coordination and reducing duplicate testing.
What’s shared: demographics, diagnoses, medications (including infusions administered here), lab/diagnostic results, and visit summaries.
Your choice: Contexture generally operates opt-out. You may opt out at any time, without affecting treatment, by contacting our Privacy Officer or Contexture directly. Opting out won’t remove information already shared.
SUBSTANCE USE DISORDER RECORDS (42 CFR PART 2)
- Part 2-protected records require your separate written consent before use or disclosure for treatment, payment, or operations — more stringent than HIPAA.
- You may restrict redisclosure; these records generally can’t be used against you in legal proceedings without your consent or a court order.
- You may opt out of fundraising communications that use Part 2 records.

- Ask us to limit certain uses or sharing
- Get a list of disclosures we’ve made
- Get a paper copy of this notice
- Name someone to act on your behalf
- File a complaint (see Complaints, right)
OUR RESPONSIBILITIES & CHANGES TO THIS NOTICE
We must protect your information, notify you of a breach affecting it, follow this notice, and get your written authorization for other uses. We may change this notice; updates will be posted at each clinic and on our website.
COMPLAINTS & CONTACT
Contact our Privacy Officer, or the U.S. Dept. of Health and Human Services, Office for Civil Rights (hhs.gov/ocr/privacy/hipaa/complaints). You will not be retaliated against for filing a complaint.
